Care Advisor

Job Summary

A company is looking for a Care Advisor, Care Concierge.

Key Responsibilities

• Assist members with completing assessments and recommending service plans based on their needs
• Coordinate and schedule services while following up with members through outbound calls
• Support members in accessing community resources such as homecare, meals, and transportation

Required Qualifications

• High School Diploma preferred
• Experience in Public/Community Health or related field preferred
• Experience in Social Services, Care Coordination, or Healthcare Navigation is a plus
• Must be 18 years of age or older
• Computer proficiency and knowledge of clerical functions are required
