Job Summary

A company is looking for a Care Advisor, Care Concierge.

Key Responsibilities

Assist members with completing assessments and recommend service plans based on their needs
Coordinate and schedule services while helping members access community resources
Handle inbound and outbound calls to follow up with members and provide support

Required Qualifications

High School Diploma preferred
Experience in Public/Community Health or related fields preferred
Experience in Social Services, Care Coordination, or Healthcare Navigation is a plus
Must be 18 years of age or older
Computer proficiency and knowledge of clerical functions required
