Job Summary

A company is looking for a Care Advisor, Care Concierge.

Key Responsibilities

Assist members with assessments and recommend service plans based on their needs
Coordinate and schedule services while providing follow-up support to members
Help members access community resources related to homecare, meals, transportation, and more

Required Qualifications

High School Diploma preferred
Experience in Public/Community Health or related fields preferred
Experience in Social Services, Care Coordination, or Healthcare Navigation preferred
Must be 18 years of age or older
Computer proficiency and knowledge of clerical functions
