Job Summary

A company is looking for a Care Coach I to empower caregivers through support and education.

Key Responsibilities

• Engage and educate caregivers via phone and digital technology to enhance their wellbeing
• Support multiple caregiver programs focusing on resource navigation and care coordination
• Collaborate with the coaching team to address caregiver challenges and document interactions

Required Qualifications

• 2+ years of relevant experience in care/disease management with older adults or individuals with disabilities
• Comprehensive knowledge of Medicaid, Medicare, and Managed Care
• Strong working knowledge of case management systems and community service delivery
• Ability to multitask in a fast-paced environment
• Proficiency in Microsoft Office suite
