Job Summary

A company is looking for a Care Coach II to support family caregivers and their care recipients.

Key Responsibilities

• Engage and educate caregivers through phone and digital technology to improve their wellbeing
• Manage a caseload and assess caregiver and care recipient needs to create and achieve care plan goals
• Document coaching activities and collaborate with stakeholders to identify problems and solutions

Required Qualifications and Education

• 4+ years of experience in care planning or case management
• Experience working with the elderly or adults with disabilities
• Solid understanding of care planning and case management principles
• Proven experience accessing community resources
• Ability to navigate mobile applications and multitask in a fast-paced environment
