Job Summary
A company is looking for a Care Coach II to support family caregivers through empathy, technology, and proven care methods.
Key Responsibilities
- Engage and educate caregivers via phone and digital technology to improve their well-being
- Manage a caseload and assess caregiver and care recipient needs to achieve care plan goals
- Document coaching activities and collaborate with stakeholders to identify solutions and improve processes
Required Qualifications
- 4+ years of experience in care planning or case management
- Direct experience supporting seniors or adults with disabilities
- Remote case management experience is highly desired
- CCM, CMC, or CGCP certification is highly desirable
- Ability to navigate mobile applications and multitask in a fast-paced environment
