Job Summary

A company is looking for a Care Coach II to support family caregivers through empathy, technology, and proven care methods.

Key Responsibilities
  • Engage and educate caregivers via phone and digital technology to improve their well-being
  • Manage a caseload and assess caregiver and care recipient needs to achieve care plan goals
  • Document coaching activities and collaborate with stakeholders to identify solutions and improve processes
Required Qualifications
  • 4+ years of experience in care planning or case management
  • Direct experience supporting seniors or adults with disabilities
  • Remote case management experience is highly desired
  • CCM, CMC, or CGCP certification is highly desirable
  • Ability to navigate mobile applications and multitask in a fast-paced environment
