Job Summary

A company is looking for a Care Coach II SCAN.

Key Responsibilities

• Engage and educate caregivers through phone and digital technology to empower their care management
• Assess caregiver and care recipient needs, assisting in creating care plan goals and coordinating resources
• Document coaching activities and provide feedback for process improvement and quality outcomes

Required Qualifications

• Bachelor's Degree with 4-6 years of experience in care management or coordination
• Experience with older adults, dementia, and chronic condition management is required
• Managed care or health plan experience is strongly preferred
• Multilingual skills, specifically in Spanish, are required
• Must be able to work 8-5 PST / 11-8 EST and participate in an on-call rotation
