Care Coach II

Job Summary

Supporting family caregivers through empathy and technology, the full-time Care Coach II will manage complex cases, provide ongoing coaching and education, and collaborate with interdisciplinary teams remotely.

Key responsibilities

• Engage and educate caregivers via phone and digital technology to empower them in managing their health and well-being
• Assess caregiver and care recipient needs, assisting in the creation and achievement of care plan goals
• Document coaching activities and collaborate with stakeholders to enhance caregiver engagement and care plans

Required qualifications

• Bachelor's Degree with 4-6 years of experience in care management, care coordination, or health coaching
• Experience with older adults, dementia, and chronic condition management is required
• Managed care or health plan experience is strongly preferred
• Fluency in Spanish is required
• Proven ability to navigate and teach the use of mobile applications
