Job Summary
Supporting family caregivers remotely, the full-time Care Coach II will engage and educate caregivers through technology and proven care methods, managing complex cases while collaborating with case managers to enhance care plans and outcomes.
Key responsibilities
- Engage, motivate, and coach caregivers via phone and digital technology to improve their psychosocial and physical health
- Manage a caseload, leveraging technology for effective coaching and information referral
- Assess caregiver and care recipient needs, assisting in the creation and achievement of care plan goals
Required qualifications
- 4+ years of experience in care planning or case management
- Experience working with the elderly or adults with disabilities
- Bachelor's degree
- Comprehensive knowledge of industry concepts and principles
- Proven experience accessing and locating community resources
