Job Summary
Supporting family caregivers through empathy and technology, the full-time Care Coach II will manage complex cases, providing education and guidance via phone and digital platforms while collaborating with case managers across multiple states.
Key responsibilities
- Engage and educate caregivers to empower them in managing their psychosocial and physical health
- Manage a caseload and utilize technology for effective coaching and information referral
- Assess caregiver and care recipient needs, assisting in the creation and achievement of care plan goals
Required qualifications
- 4+ years of experience in care planning or case management
- Experience working with the elderly or adults with disabilities is highly desired
- Comprehensive knowledge of industry concepts and principles related to clinical topics
- Multilingual skills with Spanish fluency required
- Ability to work 11-8 EST/8-5 PST
