Care Coach - Seasonal Position
Job Summary
Seeking compassionate and organized professionals, the seasonal Care Coach - Work-From-Home Opportunity will assist adult patients, including the elderly and those with chronic conditions, in understanding their healthcare needs and navigating available resources, with a commitment through June 2027.
Key responsibilities
- Conduct health assessments and gather relevant patient information
- Help patients understand their health benefits and connect them with appropriate services
- Educate and guide patients as they navigate healthcare services while documenting interactions accurately
Required qualifications
- Associate degree or higher in Allied Sciences, Social Sciences, Social Work, Human Services, or a related field with 1 year of direct adult patient care experience, or an Associate degree in any discipline with 2 years of direct adult patient care experience
- Relevant patient care experience in healthcare settings such as long-term care, hospitals, or managed care
- Experience supporting elderly and/or chronically ill populations is preferred
- Proficiency with Microsoft Word, Excel, Outlook, and electronic record systems
- Strong critical-thinking and problem-solving skills
