Care Community Support Specialist

Job Summary

A company is looking for a Care Community Support Specialist.

Key Responsibilities

• Recover no contacts and disenrollment, and complete Patient-Reported Outcome Measures (PROMS)
• Conduct outreach to address HEDIS-related gaps and collaborate with coaches and quality specialists
• Maintain accurate records of member contacts, scheduled sessions, and program performance data

Required Qualifications

• Bachelor's degree required; RN, LPN, or Nationally Certified Coach preferred
• 3+ years of experience in health, wellness, or community health settings required
• Equivalent education and/or experience may be considered in lieu of a degree
