Job Summary

A company is looking for a Care Concierge Coordinator to enhance the client experience through empathetic communication and efficient scheduling.

Key Responsibilities
  • Conduct outbound calls to newly referred clients and schedule appointments with occupational therapists
  • Manage inbound client inquiries across various communication channels and document interactions in Salesforce
  • Communicate with health plan and referral partners, troubleshooting inquiries and maintaining professionalism
Qualifications
  • 2+ years of experience in a customer-facing role involving high-volume phone communication or care coordination
  • Ability to build rapport quickly, particularly with older adults or high-needs populations
  • Proficiency in Salesforce and familiarity with remote, tech-enabled environments
  • Highly organized with strong follow-through in managing multiple client interactions
  • Experience in healthcare, managed care, or social services preferred
