Job Summary

A company is looking for a Care Connections Representative.

Key Responsibilities:

• Conduct outbound calls to schedule appointments with Nurse Practitioners
• Remove barriers for members and escalate high impact issues
• Accurately schedule appointments to meet key metrics based on scripts and goals

Required Qualifications:

• High School Diploma or equivalency
• Working knowledge of Microsoft Office or comparable software
• Ability to multi-task and manage time effectively
• Bilingual in English/Spanish
• Associate's degree or equivalent combination of education and experience preferred
