Care Connections Specialist

Job Summary

Providing essential support to patients, the full-time Care Connections Specialist will answer incoming calls, gather information, and coordinate with clinical teams in a remote setting, working from 2:45pm to 11:15pm, including rotating weekends and holidays.

Key responsibilities

• Answer incoming calls on behalf of clinics and assist patients during and after clinic hours
• Gather patient information to determine appropriate next steps and schedule appointments
• Document patient concerns and collaborate with clinical teams for urgent needs

Required qualifications

• High School Diploma or GED required
• Minimum six months of experience in customer service or customer relations required
• Call center and healthcare experience preferred
• Proficient in Microsoft Office and ability to multitask across various software platforms
• Knowledge of basic medical terminology preferred
