Care Connector

Job Summary

Supporting the daily operations of integrated care management programs, the remote Care Connector will process calls from members and providers, identify care gaps, and assist members in accessing healthcare resources and benefits.

Key responsibilities

• Process calls from members and providers in a contact center environment
• Identify members with Care Gaps/HEDIS-related health conditions and facilitate access to care
• Provide educational materials and implement strategies to enhance healthcare adherence

Required qualifications

• High School Diploma or equivalent required
• Minimum two years of recent work experience in a customer service environment
• Experience in member service or customer service telephone roles, preferably in managed care
• Preferred training or certification as a Medical Assistant, Home Health Aide, or Nursing Assistant
• Familiarity with medical terminology is preferred
