Job Summary
To support a growing telehealth program, the full-time Care Coordination Specialist will manage patient engagement, triage, and case management while working remotely with patients in Pennsylvania, New Jersey, New York, and Delaware.
Key responsibilities
- Serve as the first point of contact for patients, assessing urgency and triaging requests to the appropriate team
- Manage an active caseload by developing individualized care plans and maintaining consistent communication with patients
- Document patient interactions and collaborate with the clinical team to ensure high-quality care delivery
Required qualifications
- 2+ years of experience in care navigation, case management, or patient-facing customer support in a healthcare setting
- Working knowledge of community resources and systems in PA, NJ, NY, and/or DE
- Proficiency in using EMR and care management platforms
- High school diploma or GED required; associate's or bachelor's in social work, public health, or human services strongly preferred
- Ability to manage a caseload and prioritize tasks across multiple communication channels
