Job Summary
Working remotely in Virginia, the full-time Care Coordination Technician will provide direct support to Care Coordination staff by managing telephone triage, data entry, and member outreach to enhance health outcomes.
Key responsibilities:
- Coordinate care by making routine and ad hoc calls to members in compliance with their plan of care
- Assist Care Coordinators in arranging services and scheduling Health Risk Assessments and other member-related activities
- Extract and distribute reports, ensuring timely follow-up on medical equipment and post-discharge services
Required qualifications:
- High School diploma or equivalent
- Two years of administrative experience in a clinical setting
- Knowledge of medical terminology and Medicaid/Medicare benefits
- Familiarity with Virginia regulatory compliance standards
- Experience in scheduling and navigating calendars for multiple providers
