Job Summary

Working remotely in Virginia, the full-time Care Coordination Technician will provide direct support to Care Coordination staff by managing telephone triage, data entry, and member outreach to enhance health outcomes.

Key responsibilities:
  • Coordinate care by making routine and ad hoc calls to members in compliance with their plan of care
  • Assist Care Coordinators in arranging services and scheduling Health Risk Assessments and other member-related activities
  • Extract and distribute reports, ensuring timely follow-up on medical equipment and post-discharge services
Required qualifications:
  • High School diploma or equivalent
  • Two years of administrative experience in a clinical setting
  • Knowledge of medical terminology and Medicaid/Medicare benefits
  • Familiarity with Virginia regulatory compliance standards
  • Experience in scheduling and navigating calendars for multiple providers
