Job Summary

Key Responsibilities

• Engage and enroll patients through telephonic outreach, serving as their primary point of contact
• Develop and implement care plans in collaboration with the interdisciplinary care team, ensuring seamless transitions of care
• Identify and address social determinants of health, connecting patients to necessary resources and advocating for their needs

Required Qualifications

• 3 years of experience in healthcare coordination or a related field
• Experience with a team-based approach to care
• Strong knowledge of community resources
• Ability to prioritize tasks and work effectively in a remote environment
• Intermediate skills with MS Office Suite, including Outlook and Teams
