Job Summary

A company is looking for a Care Coordinator.

Key Responsibilities

• Coordinate with the Care Team to ensure resources for member enrollment and maintenance in the IPC Copay Assistance Program
• Maintain patient database for follow-up, tracking, and reporting
• Assist patients with enrollment in the manufacturer's program and review claims as needed

Required Qualifications

• High School Diploma and/or GED with 2 to 3 years of relevant experience
• Proficient with MS Office Suite
• Professional telephone demeanor
• Ability to maintain a high level of confidentiality
• Preferred: Certified Pharmacy Technician (CPhT)
