Job Summary

A company is looking for a Care Coordinator to serve as a patient advocate and point of contact throughout the continuum of care.

Key Responsibilities
  • Support patient experience from preadmission through the continuum of care
  • Coordinate admissions, including screening patients and confirming insurance verification
  • Maintain communication with patients and collaborate with interdisciplinary team members regarding patient care
Required Qualifications, Training, and Education
  • High School diploma required; Bachelor's degree preferred
  • Two years of customer service and relevant experience, preferably in a healthcare setting
  • Experience with computers and proficiency in software platforms and EMRs
  • Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes preferred
  • Valid driver license preferred for Oconomowoc location, with insurable status by the company's insurance policy
