Job Summary
A company is looking for a Client & Caregiver Coordinator.
Key Responsibilities
- Act as the first point of contact for clients and families, managing inquiries and scheduling needs
- Conduct client onboarding assessments and develop initial care plans
- Support caregivers through coaching and ensure client satisfaction through regular check-ins
Required Qualifications
- At least 2 years of experience in a licensed home care organization
- At least 2 years in a care coordination role or similar
- Bachelor's degree preferred, but not required
- Ability to work 100% remotely and manage a high-volume workload
- Experience with documentation and record-keeping for continuity of care
