Job Summary

A company is looking for a Client & Caregiver Coordinator.

Key Responsibilities
  • Act as the first point of contact for clients and families, managing inquiries and scheduling needs
  • Conduct client onboarding assessments and develop initial care plans
  • Support caregivers through coaching and ensure client satisfaction through regular check-ins
Required Qualifications
  • At least 2 years of experience in a licensed home care organization
  • At least 2 years in a care coordination role or similar
  • Bachelor's degree preferred, but not required
  • Ability to work 100% remotely and manage a high-volume workload
  • Experience with documentation and record-keeping for continuity of care
