Key Responsibilities

• Serve as the first point of contact for members, providing empathetic support and directing inquiries to appropriate care team members
• Coordinate scheduling and logistics for member visits, ensuring all necessary documentation is complete and up to date
• Engage with members through proactive outreach and support, helping them navigate non-clinical resources and services

Required Qualifications

• High School diploma or equivalent
• 1-2 years of experience as a patient care coordinator or community health worker
• At least 1 year of experience with EHR/EMR systems, particularly in scheduling and insurance verification
• Experience verifying insurance
• Authorized to work and currently residing in the United States
