Job Summary

A company is looking for a Care Coordinator.

Key Responsibilities
  • Provide technical support and training for family caregivers using technology platforms
  • Coordinate care plans and support the coaching team with logistical needs
  • Serve as a liaison with healthcare providers and manage community resource databases
Required Qualifications
  • 2+ years of experience in healthcare coordination, case management, or patient navigation
  • Experience working with older adults and/or dementia populations preferred
  • Proficiency in care management software and electronic health records
  • Bachelor's degree in a related field or equivalent experience preferred
  • Certification in case management (CCM) or care coordination is a plus
