Job Summary

Key Responsibilities
- Provide technical support and training for family caregivers using technology platforms
- Coordinate care plans and support the coaching team with logistical needs
- Serve as a liaison with healthcare providers and manage community resource databases
Required Qualifications
- 2+ years of experience in healthcare coordination, case management, or patient navigation
- Experience working with older adults and/or dementia populations preferred
- Proficiency in care management software and electronic health records
- Bachelor's degree in a related field or equivalent experience preferred
- Certification in case management (CCM) or care coordination is a plus
