Care Coordinator

Job Summary

A company is looking for a Care Coordinator to serve as a patient advocate and point of contact for care recommendations.

Key Responsibilities

• Support patient experience from preadmission through the continuum of care
• Ensure timely follow-up communication with patients and facilitate internal transfers
• Develop working relationships with interdisciplinary team members to coordinate patient care

Required Qualifications, Training, and Education

• High School diploma required; Bachelor's degree preferred
• Two years of customer service experience, preferably in a healthcare setting
• Proficiency with computers and software platforms, including EMRs
• Knowledge of medical/psychiatric diagnosis and treatment processes preferred
• Valid driver's license preferred for specific locations with insurable status
