Job Summary

A company is looking for a Care Coordinator responsible for supporting Employee Assistance Program clients and managing case-related tasks.

Key Responsibilities
  • Provide support to Employee Assistance Program clients while adhering to established procedures
  • Track and follow up with assigned members, ensuring appointments are scheduled and feedback is obtained
  • Research and identify local EAP affiliate providers and care facilities based on member preferences
Required Qualifications
  • Bachelor's degree in psychology, behavioral health, health care management, liberal arts, or a related field
  • Minimum of two years of customer service, health care, or case work experience
  • Intermediate knowledge of MS Excel and Word
