Care Coordinator

Job Summary

A company is looking for a Care Coordinator 1 to serve as a patient advocate and point of contact for care recommendations.

Key Responsibilities

• Support patient experience preadmission and throughout the continuum of care
• Ensure timely follow-up with patients and facilitate internal transfers between sites
• Develop working relationships with interdisciplinary team members to ensure effective communication regarding patient care

Required Qualifications, Training, and Education

• High School diploma required; Bachelor's degree preferred
• Two years of customer service and relevant experience, preferably in a healthcare setting
• Experience with computers and proficiency with software platforms and EMRs
• Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes preferred
• Valid driver license preferred for Oconomowoc location, with insurable status by the company's insurance policy
