Care Coordinator

Job Summary

Working in a fast-paced environment, the full-time Care Coordinator will engage clients to arrange laboratory testing services, verify referral information, and maintain proactive communication to support successful testing delivery.

Key responsibilities:

• Verify and validate referral information and manage scheduled referrals
• Initiate contact with clients to provide education on the scheduling process and address any barriers
• Document actions thoroughly in the referral management system and update involved parties on referral status

Required qualifications:

• High School Diploma required; AA, AS, or college degree preferred
• 5+ years of customer service experience, including a minimum of 3 years in healthcare
• Preferred experience in pharmacy or toxicology as a laboratory technician/assistant or pharmacy technician
• Proficiency in computer-based programs
• Bilingual in Spanish is a plus
