Job Summary

Providing primary support for clients seeking mental health care, the full-time remote Care Coordinator will manage client intake, provider matching, and appointment scheduling while ensuring clients receive the necessary support throughout their care journey.

Key responsibilities

• Act as the first point of contact for clients, managing their onboarding and care coordination
• Utilize technology to connect clients with appropriate therapists and maintain ongoing support
• Conduct outreach to referred clients and assist with scheduling and communication with care team members

Required qualifications

• Experience in care coordination or a related field, or recent college graduates seeking a meaningful career change
• Strong desire to assist clients in mental health care and ability to work with individuals in vulnerable situations
• Motivated and eager to learn, with the ability to adapt to new technologies and processes
