Job Summary

To support patient care and advocacy, the full-time remote Care Coordinator will manage patient inquiries, coordinate admissions, and facilitate communication among interdisciplinary teams from Tuesday to Saturday, 10:00 am to 6:30 pm CST.

Key responsibilities:
  • Screen patients and relay program/service recommendations to patients and families
  • Ensure timely follow-up with patients and maintain open communication with guardians
  • Facilitate internal transfers between sites by managing referrals and communicating relevant information
Required qualifications:
  • High school diploma required; bachelor's degree preferred
  • Two years of customer service experience, preferably in a healthcare setting
  • Proficiency with software platforms and electronic medical records (EMRs)
  • Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes preferred
  • Valid driver's license preferred for Oconomowoc location, with insurable status required
