Care Coordinator

Job Summary

Supporting the day-to-day functions of a complex medical network, the full-time Care Coordinator will manage outreach functions, case assignments, and member communication while working remotely.

Key responsibilities

• Initiate outreach cycles and document member referral statuses within the Case Management Department
• Track and manage member needs, ensuring timely follow-up and coordination of health care services
• Provide education and support to members and their families regarding care transitions and health insurance benefits

Required qualifications

• High School diploma required; Associate's degree in a related field preferred
• 3-5 years of experience in a professional environment; healthcare experience, especially in managed care, is preferred
• Proficiency in data entry and familiarity with platforms like Casetrakker, JIVA, or MedHok
• Strong computer skills, particularly in Microsoft Office applications
• Ability to work independently and collaboratively within a team environment
