Job Summary
Serving as a patient advocate in a remote capacity, the full-time Care Coordinator will manage patient inquiries, coordinate admissions, and facilitate communication among interdisciplinary teams to ensure seamless care from recommendation to treatment.
Key responsibilities:
- Support patient experience by screening patients and relaying program recommendations
- Ensure timely follow-up and maintain open communication with patients and families
- Facilitate internal transfers and collaborate with various departments to streamline patient care
Required qualifications:
- High School diploma required; Bachelor's degree preferred
- Two years of customer service experience, preferably in a healthcare setting
- Proficiency with computers and software platforms, including EMRs
- Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes
- Familiarity with managed care and Joint Commission standards preferred
