Job Summary
Supporting the Care Management department, the full-time remote Care Coordinator will manage outreach functions, case assignments, and member follow-ups while ensuring timely coordination of healthcare services.
Key responsibilities
- Initiate outreach cycles and document member referral statuses within the electronic record
- Track and triage member needs, providing education and support for follow-up care and health management
- Communicate with external stakeholders to coordinate member care and assist with scheduling appointments and services
Required qualifications
- High School diploma required; Associate's degree in a related field preferred
- 3-5 years of experience in a professional environment; healthcare experience is highly desirable
- Proficiency in Microsoft Office and ability to learn Casetrakker, JIVA, or MedHok within 3 months
- Strong knowledge of clinical services and healthcare operations
- Ability to work independently and as part of a team in a fast-paced environment
