Care Coordinator

Job Summary

Serving as the first point of contact for patients, the part-time hourly Care Coordinator will manage inbound calls, facilitate communication between care teams, and ensure a compassionate, patient-centered experience while working remotely.

Key responsibilities

• Answer patient calls and transfer escalated issues to appropriate team members
• Identify and collect necessary documentation from patients and community providers
• Accurately document patient interactions and ensure timely follow-up on assigned tasks

Required qualifications

• High School Diploma / GED (or higher)
• Experience with escalated customer service issue resolution
• Knowledge of Google Suite products, including Google Docs, Gmail, Google Calendar, Google Slides, and Google Sheets
• Phone/Fax system experience
• EMR experience, with Athena & Epic preferred
