Job Summary
Serving as the first point of contact for patients, the part-time hourly Care Coordinator will manage inbound calls, facilitate communication between care teams, and ensure a compassionate, patient-centered experience while working remotely.
Key responsibilities
- Answer patient calls and transfer escalated issues to appropriate team members
- Identify and collect necessary documentation from patients and community providers
- Accurately document patient interactions and ensure timely follow-up on assigned tasks
Required qualifications
- High School Diploma / GED (or higher)
- Experience with escalated customer service issue resolution
- Knowledge of Google Suite products, including Google Docs, Gmail, Google Calendar, Google Slides, and Google Sheets
- Phone/Fax system experience
- EMR experience, with Athena & Epic preferred
