Care Coordinator

Job Summary

Supporting patients and caregivers navigating dementia-related care, the full-time Care Coordinator will manage outreach, documentation, and cross-team communication to ensure smooth care pathways while working Monday through Friday from 8:00 am to 4:30 pm EST with flexible scheduling options.

Key Responsibilities

• Conduct outreach to patients and caregivers to address care coordination needs and manage follow-up across a dynamic caseload
• Accurately document all interactions and maintain organized case notes in the electronic health record and tracking systems
• Communicate patient updates and flag emerging issues to nursing, intake, support, and leadership teams as necessary

Required Qualifications

• Minimum 3 years of experience in care coordination, case management, or patient navigation
• Experience working with older adults and/or patients with dementia and their caregivers
• Strong initiative and ability to manage multiple patients and priorities
• Minimum of 1 year experience working in a remote environment
• Excellent organizational and documentation skills
