Care Coordinator

Job Summary

Acting as a patient advocate, the full-time remote Care Coordinator will manage patient inquiries, coordinate admissions, and facilitate communication among interdisciplinary teams to ensure a seamless continuum of care.

Key responsibilities

• Support patient experience preadmission and throughout the continuum of care by screening patients and relaying program recommendations
• Ensure timely follow-up with patients and families, maintaining open lines of communication
• Facilitate internal transfers between sites by managing referrals and communicating pertinent information to treatment teams

Required qualifications

• High School diploma required; Bachelor's degree preferred
• Two years of customer service experience, preferably in a healthcare setting
• Proficiency with computers and software platforms, including EMRs
• Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes
• Familiarity with managed care and Joint Commission standards preferred
