Job Summary
Acting as a patient advocate, the full-time remote Care Coordinator will manage patient inquiries, coordinate admissions, and facilitate communication among interdisciplinary teams to ensure a seamless continuum of care.
Key responsibilities
- Support patient experience preadmission and throughout the continuum of care by screening patients and relaying program recommendations
- Ensure timely follow-up with patients and families, maintaining open lines of communication
- Facilitate internal transfers between sites by managing referrals and communicating pertinent information to treatment teams
Required qualifications
- High School diploma required; Bachelor's degree preferred
- Two years of customer service experience, preferably in a healthcare setting
- Proficiency with computers and software platforms, including EMRs
- Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes
- Familiarity with managed care and Joint Commission standards preferred
