Supporting the clinical practice's efficiency, the full-time Care Coordinator will manage patient navigation, eligibility checks, and scheduling while collaborating with clinical staff in a fully remote environment.

Key responsibilities

• Provide care navigation support for patients seeking providers and social services
• Monitor the member support phone line and address patient inquiries
• Manage patient scheduling, prioritization, and appointment reminders

Required qualifications

• 4+ years of healthcare administration experience, preferably in virtual care
• Familiarity with social determinants of health (SDOH) platforms and referrals
• Results-oriented with a strong ability to work cross-functionally
• Meticulous attention to detail and a passion for maternal and child health
• Comfort with ambiguity and a proactive approach to problem-solving
