Care Coordinator

Job Summary

To support a growing palliative care program, the full-time Care Coordinator will work remotely to enroll patients, connect them with providers, and ensure they receive high-quality care while acting as a trusted resource for patients and their families.

Key responsibilities

• Help patients understand the value of palliative care and enroll them in the program
• Document patient encounters and triage any patient needs to their care team
• Follow up with patients to ensure they are receiving high-quality care from Empassion's network of providers

Required qualifications

• Minimum 2+ years of relevant work experience in a customer service call center or healthcare
• Basic understanding of healthcare
• Caring and compassionate background, preferably in social work or similar fields
• Enthusiastic about providing an outstanding experience for patients and their caregivers
• Comfortable working with technology in a dynamic, startup environment
