Care Coordinator

Job Summary

Serving as a patient advocate and point of contact, the full-time remote Care Coordinator will manage patient inquiries, coordinate admissions, and ensure timely follow-up throughout the continuum of care.

Key responsibilities

• Support patient experience by relaying program information and coordinating admissions appointments
• Maintain open communication with patients and facilitate internal transfers between sites
• Develop relationships with interdisciplinary team members to ensure seamless patient care

Required qualifications

• High School diploma required; Bachelor's degree preferred
• Two years of customer service experience, preferably in a healthcare setting
• Proficiency with computers and experience with software platforms and EMRs
• Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes preferred
• Valid driver license preferred for Oconomowoc location, with insurable status required
