Care Coordinator

Job Summary

To support a growing mental health initiative, the part-time hourly Care Coordinator will manage patient engagement, coordinate care, and document progress while working remotely with a cross-disciplinary team to enhance the patient experience throughout their reproductive journey.

Key Responsibilities

• Communicate with patients to facilitate program engagement and provide ongoing support and education
• Update patient records in Electronic Health Records (EHRs) and maintain a collaborative care registry for tracking follow-up and outcomes
• Collaborate with clinic staff to ensure coordinated care and timely communication of patient needs

Required Qualifications

• Bachelor's degree and/or Master's Degree in nursing, social work, or psychology
• Strong understanding of women's mental health conditions and community resources
• Proficiency in using EHR systems and Google Suite
• Ability to manage a high volume caseload and engage patients compassionately
• Excellent organizational skills for tracking patient records and outcomes
