Care Coordinator

Job Summary

Supporting logistical patient care activities, the full-time Care Coordinator will manage remote duties including coordinating home-based care orders, collaborating with vendors, and ensuring patients receive necessary services while maintaining clear communication with the clinical team and other stakeholders.

Key responsibilities

• Works with an interdisciplinary care team to assess and address complex care needs of patients and families
• Coordinates care by supporting admission and ongoing home-based census operations and following up on orders
• Provides administrative support for tracking ongoing care activities, including appointment scheduling and vendor liaison tasks

Required qualifications

• Minimum one year of healthcare experience, preferably three years, with a focus on care coordination
• Associate's or Bachelor's degree in a health-related field is required
• Experience working with EMR and strong organizational skills in telephonic coordination
• Proficiency in Spanish is preferred
• Experience drafting organizational protocols and workflows is preferred
