Care Coordinator

Job Summary

Supporting logistical patient care activities, the full-time Care Coordinator will manage remote duties such as coordinating care orders, liaising with vendors, and ensuring patients receive necessary services while collaborating with clinical teams.

Key responsibilities

• Assess and address the complex care needs of patients and families as part of an interdisciplinary care team
• Coordinate care by supporting admission operations and following up on orders for home-based services
• Provide administrative support for tracking ongoing care activities, including appointment scheduling and equipment coordination

Required qualifications

• Minimum one year of healthcare experience, with three years preferred, including care coordination knowledge
• Associate's or Bachelor's degree in a health-related field is required
• Experience working with EMR and strong telephonic coordination skills
• Ability to identify data elements for quality improvement initiatives
• Spanish speaking proficiency/fluency is preferred
