Care Coordinator

Job Summary

Supporting logistical patient care activities, the full-time Care Coordinator will manage remote tasks related to home-based care, coordinate with vendors, and ensure patients receive necessary services based on care team orders.

Key responsibilities

• Assess and address the complex care needs of patients and families as part of an interdisciplinary care team
• Coordinate care by supporting admission and ongoing home-based census operations, including tracking orders and scheduling appointments
• Communicate care plans and assessments effectively to front-line teams while maintaining accurate records of interventions

Required qualifications

• Minimum one year of healthcare experience, with three years preferred, including care coordination and working with clinical staff
• Associate's or Bachelor's degree in a health-related field is required
• Experience with EMR systems and strong organizational skills in telephonic coordination
• Proficiency in Spanish is preferred
• Experience drafting organizational protocols and workflows is preferred
