Care Coordinator

Job Summary

Supporting logistical patient care activities, the full-time Care Coordinator will manage remote tasks such as coordinating home-based care orders, liaising with vendors, and ensuring patients receive necessary services while collaborating with clinical teams.

Key responsibilities

• Works with an interdisciplinary care team to assess and address complex patient care needs
• Coordinates care by supporting admission processes and tracking ongoing home-based census operations
• Provides administrative support for care activities, including appointment scheduling and vendor liaison duties

Required qualifications

• Minimum one year of healthcare experience, with three years preferred
• Experience with care coordination and knowledge of EMR systems
• Associate's or Bachelor's degree in a health-related field
• Proficiency in Spanish is preferred
• Experience drafting organizational protocols and workflows is preferred
