Care Coordinator

Job Summary

Supporting logistical patient care activities, the full-time Care Coordinator will manage remote tasks such as coordinating home care orders, liaising with vendors, and ensuring patients receive necessary services while collaborating with clinical teams.

Key responsibilities

• Works as part of an interdisciplinary care team to assess and address patient and family care needs
• Coordinates care by supporting admission and ongoing home-based census operations and following up on orders
• Provides administrative support for tracking care activities, including scheduling and vendor coordination

Required qualifications

• Minimum one year of healthcare experience, with three years preferred, including care coordination
• Associate's or Bachelor's degree in a health-related field is required
• Proficiency in Spanish is preferred
• Experience working with EMR and strong organizational skills in telephonic coordination
• Ability to draft organizational protocols and workflows
