Care Coordinator

Job Summary

Supporting logistical patient care activities, the full-time Care Coordinator will manage remote coordination of home-based care services, assist with orders, and collaborate with clinical teams and vendors to ensure quality patient outcomes.

Key responsibilities

• Works with an interdisciplinary care team to assess and address the complex needs of patients and families
• Coordinates care by supporting admission processes and tracking ongoing home-based operations
• Provides administrative support, including appointment scheduling and vendor liaison activities

Required qualifications

• Minimum one year of healthcare experience, with three years preferred
• Experience in care coordination and familiarity with EMR systems
• Associate's or Bachelor's degree in a health-related field
• Spanish speaking proficiency/fluency is preferred
• Experience drafting organizational protocols and workflows
