Care Coordinator

Job Summary

Serving as a patient advocate, the full-time remote Care Coordinator will manage patient communications, coordinate admissions, and facilitate internal transfers while ensuring a seamless continuum of care.

Key responsibilities:

• Support patient experience by relaying program recommendations and coordinating admissions appointments
• Maintain timely follow-up with patients and families, ensuring open communication throughout the care process
• Develop relationships with interdisciplinary team members to facilitate effective patient care and internal transfers

Required qualifications:

• High School diploma required; Bachelor's degree preferred
• Two years of customer service experience, preferably in a healthcare setting
• Proficiency with computers and experience with software platforms and EMRs
• Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes preferred
• Valid driver's license preferred for Oconomowoc location, with insurable status required
