Key Responsibilities

• Perform telephonic outreach to enroll and engage patients in clinical programs
• Develop and implement care plans addressing patients' unique needs and facilitate transitions of care
• Conduct assessments to identify social determinants of health and connect members to appropriate resources

Required Qualifications

• 3 years of experience in healthcare coordination or community health work
• Experience with a team-based approach to care
• Excellent knowledge of community resources
• Ability to prioritize tasks and work effectively in a remote environment
• Intermediate skills with MS Office Suite, including Outlook and Teams
