Care Coordinator - CKD Navigation

Job Summary

To support patients with chronic kidney disease, the remote Care Coordinator - CKD Navigation will manage patient engagement, care coordination, and resource navigation, ensuring comprehensive support and improved health outcomes through collaboration with an interdisciplinary team.

Key responsibilities

• Perform telephonic outreach to enroll and engage patients in clinical programs, serving as the primary point of contact
• Develop and implement care plans in collaboration with the Nurse Care Manager, addressing the unique needs of patients
• Conduct assessments to identify social determinants of health and connect patients to necessary resources for improved care

Required qualifications

• 3 years of experience in healthcare coordination or community health work
• Experience with a team-based approach to care
• Strong knowledge of community resources and services
• Intermediate skills with MS Office Suite, including Outlook and Teams
• Ability to work effectively in a remote environment with required internet speeds
