Job Summary

A company is looking for a Care Coordinator - Family Support.

Key Responsibilities

• Educate families on program details and available services
• Communicate with families to review funding options and provide information on other supports
• Develop relationships with service providers to connect families with local resources

Required Qualifications

• Diploma or Bachelor's Degree in a relevant field with a minimum of 3 years of experience in human or social services
• Strong commitment to a family-centered and person-directed approach
• Proven experience working with children and youth on the autism spectrum and their families
• Thorough understanding of the developmental services sector and community-based resources
• Fluency in written and oral French is strongly preferred
